Some providers may see automatic downcoding of E/M office visit codes starting October 1, 2025
Starting October 1, 2025, Cigna will automatically downcode levels four and five evaluation and management (E/M) office visit codes for certain providers if the information submitted (e.g., diagnosis or documentation) does not clearly support the complexity of the billed CPT E/M codes.
